Instructions after log-in:

1. Only fill out the non-technical questions on Case Consideration Questionnaire, Part 1 to begin.

2. You will be advised if and when to fill out any of the other questionnaires.

Case Consideration Questionnaire Part 2 contains questions of a technical nature and will probably require the assistance of legal counsel, especially the link specific to the nature of your claim.

Since the answers to those particular questions play a significant role in the evaluation of your claim, the assistance of legal counsel will provide more reliable information. If you do not have your own attorney, any of the attorneys listed on our site under Submitting Attorneys for the Claimant will assist you at no charge in completing Case Consideration Questionnaire, Part 1 and then Case Consideration Questionnaire, Part 2, if necessary.

We realize the questionnaires are somewhat lengthy and complicated, but the information we request is necessary to make a preliminary determination about proceeding to the next level with your case. Please be certain to answer all questions asked on the Case Consideration Questionnaire, Part, 1 to enable us to process the information and to determine whether you are to complete Case Consideration Questionnaire, Part 2.

If you do not know the answer, check the box for such a response.

If a case meets LFMG’s preliminary guidelines and is eligible for consideration at the next step of the case selection review process, you will be notified and asked to complete Case Consideration Questionnaire, Part 2 and relevant links for the case formally submitted by the attorney who will represent your interests throughout the remainder of the selection process and, if approved by the LFMG Independent Case Selection Review Panel, to the conclusion of case litigation.

You may select any attorney you choose from Submitting Attorneys for the Claimant section of our website to formally submit the case. These attorneys have no affiliation or relationship with us. They are entirely independent.
